7 Signs Your Metabolism Needs Support in Midlife (Weight Gain Is Only One of Them)

Weight gain isn’t the only sign your metabolism is changing in midlife. Here are 7 signs to watch for, and why I look at the full terrain before recommending a GLP-1.

If you’re in your 40s or 50s and your body suddenly feels different, you’re not imagining it. Maybe the weight is showing up around your middle even though nothing else changed. Maybe you’re exhausted, your cravings are louder, or you just don’t feel like yourself.

And someone, somewhere, has told you: “You’re getting older. Eat less, move more. Or just accept it.”

Metabolism is more complicated than calories in versus calories out, and it’s about far more than the number on the scale. Research shows the menopause transition brings changes in body composition, visceral fat, lean muscle, insulin sensitivity, and inflammation, often before the scale moves at all.

So instead of asking “how do I lose weight,” I want women asking a better question: what has changed in my body, and what does it need now?

Here are the 7 signs I pay attention to first.

What’s Actually Happening in Your Body

1. Your body composition is changing even if the scale isn’t. “My weight hasn’t changed much, but my body looks completely different” is one of the most common things I hear. Waist expanding, more fat through the abdomen and thighs, muscle definition disappearing, cellulite showing up out of nowhere. Declining estrogen is linked to a shift in where fat gets stored, including more visceral fat, along with changes in lean mass. If you’re Googling “menopause belly fat,” you’re often finding advice built for weight loss when what your body actually needs is more calories and more nutrient density, not less.

2. You’re tired even though you’re doing everything right. You eat well, you exercise, you sleep close to 7 to 8 hours (even if you’re waking up through the night). And you’re still exhausted. Fatigue in midlife has many possible drivers: sleep disruption, inadequate nutrition, thyroid function, iron levels, hormonal shifts, stress, and blood sugar swings. But there’s a piece that gets missed: your mitochondria need to efficiently produce and use energy. Metabolic health isn’t just calorie burn. It’s how well your body responds to insulin, uses fuel, and maintains muscle. Telling a tired midlife woman to eat less and move more sometimes makes this worse, not better.

3. Cravings, crashes, or shifting blood sugar. Irritable by 3pm. Craving something sweet every afternoon. Crashing after meals. Insulin sensitivity shifts quietly during the menopause transition, and most of us are never told we need to adjust for it. Cortisol and insulin are directly linked, which is why chronic stress on top of a clean diet can still produce blood sugar problems. Labs like fasting glucose, A1c, fasting insulin, and triglycerides can offer real information here, and the goal is to catch the pattern early, not wait until something is officially “wrong.”

4. You’re losing muscle, strength, or exercise capacity. Workouts feel harder. Recovery takes longer. You’ve lost definition in your arms and legs. Muscle is metabolic tissue: it supports glucose disposal, insulin sensitivity, bone health, and healthy aging, and women become more vulnerable to losing it as they age. Losing 20 pounds while also losing muscle isn’t the metabolic win the scale makes it look like. The real questions are whether you’re preserving muscle, eating enough protein, strength training, recovering well, and adequately nourished.

5. Sleep, stress, and recovery have changed. Poor sleep affects appetite regulation, glucose metabolism, food choices, and workout recovery, and midlife often stacks hormonal changes, hot flashes, work, kids, aging parents, and stress all at once. Layering intense workouts and aggressive calorie restriction on top of an already stressed system rarely helps. Morning light, walking, strength training (without overtraining), eating enough of the foods your body needs (this varies because some women are fast oxidizers and many women are very slow oxidizers…they’re nutrient needs are different), and real nervous system support matter as much as any supplement trending this month.

6. Your digestion has changed. Bloating, constipation, new food intolerances, reflux. These symptoms have many causes and aren’t automatic proof your metabolism is “broken,” but digestion and nutrition can’t be separated from metabolic health. If you’re eating well but not tolerating it, or your appetite has dropped so low you can’t get enough protein and fiber, that matters too.

7. The strategies that used to work, don’t anymore. At 30, cleaning up your diet for two weeks worked. Skipping breakfast and living on coffee somehow worked. Now you’re doing more and getting less back. That doesn’t mean your body failed you. It means your physiology changed, and it’s time for a more sophisticated conversation.

Why I Don’t Start With the Scale

When a woman comes to me with unexplained weight gain, fatigue, cravings, or muscle loss, I look at the bigger picture: glucose and insulin regulation, thyroid markers, iron and nutrient status, lipids, protein and fiber intake, muscle mass, sleep, stress, digestion, hormones, and mineral patterns. Blood work gives real information, but I often start with Hair Tissue Mineral Analysis (HTMA) to look at mineral relationships, stress patterns, digestion and detox capacity and blood sugar cues. HTMA often shows us where the blocks are and how things are moving into and out of hte cells. This is having a big impact on metabolic function. What’s most common in midlife is hearing “your blood work looks normal” so unless there is a clear reason to start with bloodwork…I start with HTMA. This shows us what’s happening before it gets bad enough to actually show up on bloodwork. One isn’t necessarily better than the other, but in midlife, HTMA is usually a better starting point, especially for weight and metabolic issues that aren’t sever enough to show up on as something like prediabetes but definitely have you questioning what the hell is happening in your body. The point isn’t to run every test available. It’s to stop guessing.

What Standard Advice Misses: Before a GLP-1, Look at the Terrain

Here’s the part of the GLP-1 conversation I don’t think gets enough attention: what condition is the body in before the medication gets added? I call this looking at the terrain.

I’ve seen women spend hundreds of dollars a month on a GLP-1 and lose fewer than five pounds. I’ve also seen women start one while already exhausted, constipated, under-eating protein, losing muscle, and dealing with real blood sugar dysregulation. A GLP-1 can be a genuinely powerful tool. It doesn’t make the rest of your physiology irrelevant.

Before adding a metabolic tool like this, I want to understand a woman’s baseline: her blood sugar regulation, whether she’s eating enough protein to preserve muscle, whether she’s strength training, how her digestion and hydration look, what her sleep and energy are like, and what her labs actually show. Are we building a body that’s nourished and supported, or asking an already depleted one to eat even less? This doesn’t mean we have to start with test, but we at least start with an extensive lifestyle and healthy intake. And often, we get a baseline before adding the GLP1 so we know how to support your body while you’re on it AND we can retest to see what needles we moved beyond the scale. These are real metabolic medications that are having impacts far beyond the scale and they’re worth measuring!

One of the primary effects of GLP-1 medications is reduced appetite, along with slower gastric emptying and changes in glucose regulation. For many people that’s exactly why they work. But if a woman goes into that already eating too little protein, already constipated, already losing muscle, and we substantially reduce how much she eats, there are now fewer chances to get protein, fiber, and the nutrients she needs. That’s why I want to build the foundation, protein intake, strength training, fiber, hydration, blood sugar stability, sleep, before adding the tool whenever possible, or at least at the same time, not after something breaks.

Where GLP-1s Actually Fit

I don’t think GLP-1s should be demonized, and I don’t think the conversation should start and end with “how do we make you eat less.” For an appropriately screened person, they can reduce appetite, improve glycemic control, and produce real weight loss. But muscle, blood sugar, body composition, nutrition, digestion, sleep, and metabolic function still matter whether or not you’re using one. If your appetite drops dramatically, what you do eat matters even more.

The goal isn’t to be pro or anti GLP-1. It’s to build the healthiest metabolic terrain possible, then use the right tool for the right person.
